Eligibility
Requirements:
Routine immunizations are
available during these clinic hours to all eligible clients; PLEASE refer to the
criteria above for confirming eligibility. Eligibility will be determined
through completion of a screening questionnaire; NOTE, falsely claiming
eligibility is fraud and abuse. The social security card for the person
receiving immunizations is required to be presented during the initial visit to
the Health Department.
The following information is required for
registration:
- A current
immunization record must be presented each time a person comes to the health
department for immunizations. (This applies to all persons less than 27 years of
age.) We prefer an immunization record for all persons receiving
immunizations.
- A Social Security card must be
presented for all participants. For those not having a social security number.
the county of birth must be stated.
- A parent or guardian must
accompany any child under 18 years of age to the clinic.
- Child must be free of fever
> 24 hours.
Administration Fees:
(all are subject to change
at anytime)
Children: $14.00 per child for 1 shot and $29.00 per child
for 2 shots or more for qualified patients per visit. We also accept current Medicaid Card
Medicare accepted for Influenza vaccine
only.
*$2.00 fee for duplicate copy of an
immunization record.
No one, qualifying for STATE SUPPLIED
VACCINE, is refused immunization services due to an inability to pay the
administration fee(s).
